Hello guys. I started playing promods some versions ago and i have seen this words appearance sometimes.
I supose that the rebuilt is making the city from scratch, but the Refresh and "Improved city" dont know what they mean exactly

Rebuilt - partly/fully redone city/road from scratch
Refresh/Improved - Major/minor improvements, such as texture replacements, better road signs, better roads etc.
